For Offline Signup:

1. Click Here to download the application form.

2. Print a copy.

3. Complete the Application form (N.B. Signature required on both pages)  *Please include your e-mail address. 

4. Fax both completed pages to our Sydney office on (02) 8765 1317    

5. Check your e-mail regularly for your welcome letter and ID number.


*If you do not have an e-mail address, simply note this on your application form, and we will arrange an alternative form of communication with you.
